The mitochondrial toxicity of nucleoside reverse transcriptase inhibitors (NRTIs) is due to the inhibition of mitochondrial DNA (mtDNA) polymerase gamma (Pol gamma), resulting in a blockade of mtDNA replication and subsequent disruption of cellular energetics.
