This is the first report of the unusual syndrome of persistent polyclonal B-cell lymphocytosis occurring in monozygotic twins. The syndrome is characterized by a lymphocytosis, with circulating atypical, binucleated lymphocytes, mild splenomegaly and raised serum IgM.
