We describe an 18-year-old woman with primary antiphospholipid syndrome whose monozygotic twin sister is as yet an asymptomatic carrier of the lupus anticoagulant, anticardiolipin, and anti-beta 2 glycoprotein I (beta 2GPI) antibodies.
