<title>Abstract</title> <p><bold>Background: </bold>Hereditary spastic paraplegias type 5 (SPG5) is an inherited neurodegenerative disease with 27-hydroxycholesterol abnormal accumulation. Imaging and pathologic manifestations remain poorly understood due to the rare incidence.
