Idiopathic nephrotic syndrome (INS) is characterized by diffuse foot process effacement on electron microscopy and minimal changes (called minimal change nephropathy [MCN]), focal segmental glomerular sclerosis (FSGS), or the mesangial variant with proliferation on light microscopy.
