Matching the exposure window to the response

Suppose two dosing occasions have the same recorded dose, but the response differs several hours later despite similar drug concentrations measured at that later time. If earlier exposure was not measured, that comparison leaves the exposure timeline incomplete. What timing evidence would distinguish a delayed response to an earlier exposure peak from a response that tracks concentration at the time it is measured?
