<title>Abstract</title> <p> Background Hemophagocytic lymphohistiocytosis (HLH) is a fatal hyperinflammatory syndrome frequently triggered by infections in immunocompromised hosts. In HIV patients, upon initiation of antiretroviral therapy (ART), tuberculosis-associated immune reconstitution inflammatory syndrome (TB-IRIS) can overlap and present with fever, cytopenias, and hepatosplenomegaly.
