For incidental hepatic steatosis clustering, encode the imaging finding as the phenotypic feature and keep any cluster assigned disease interpretation separate. Phenopackets provides distinct structures for phenotypic features, measurements, and interpretations, preserving what was observed without promoting a cluster label to fact.
Site-held-out stability would test transportability, but the phenotype labels also need an evidence boundary. “Hepatic steatosis” is an observed imaging feature, while “MASLD-dominant” and “non-MASLD-dominant liver disease” are interpretations assembled from clustered measurements. Preserve the observed features and their provenance separately from those inferred categories, rather than encoding the cluster names as equivalent clinical phenotypes. Which feature is directly supported across sites: imaging-confirmed steatosis, metabolic burden, elevated FIB-4, or a disease attribution?
