Keep cluster labels separate from observations

For incidental hepatic steatosis clustering, encode the imaging finding as the phenotypic feature and keep any cluster assigned disease interpretation separate. Count who never reaches clustering

The study required liver enzyme measurements before clustering, so transportability reporting should count patients excluded at that gate separately from later assignment problems. Rural EDs and patients with sparse records remain outside validation unless both losses and cluster stability are reported there.

Treat the LD panel as part of the credible set

SuSiEx explicitly models population-specific allele frequencies and LD patterns, which makes the ancestry inputs part of the fine-mapping specification rather than background metadata. Its reported schizophrenia analysis used in-sample LD for European and East Asian ancestry groups. For an incidental locus, I’d compare credible-set membership and posterior inclusion probabilities under the primary LD matrix and at least one ancestry-matched alternative. Which GWAS ancestry, LD source, and credible-set probability threshold produced the reported set?

The conclusion could reverse if the region contains multiple causal signals, if the molecular and phenotype datasets use mismatched ancestry, or if the selected tissue does not represent the relevant liver process. Prior sensitivity is necessary, but conditional or multi-signal analysis is the sharper check because marginal associations can conceal distinct signals in LD. For each claimed shared signal, were both datasets ancestry matched and conditionally resolved before tissue relevance was interpreted?

If a later Mendelian randomization analysis treats one cluster as the exposure, instrument strength should be assessed against that derived assignment rather than against a single component trait. A weak association can amplify bias from invalid instruments, while a strong association does not establish the exclusion restriction. Variants associated with diabetes, adiposity, lipids, or liver enzymes could reach the outcome through pathways already embedded in the phenotype definition. Which assumption is less defensible here: relevance for cluster membership, or exclusion of direct metabolic pathways?

The hepatic steatosis study pooled adults from five emergency departments and reported three phenotypes, including a small non-MASLD dominant group with higher FIB-4 values. That multisite sample does not by itself show that cluster definitions or frequencies transport to another health system. External validation should preserve the original feature definitions and report cluster assignment, missingness, and clinically relevant contrasts by site. A sharper target would be emergency departments serving different racial and ethnic groups, rural populations, and patients with limited longitudinal records. Which of those populations remains entirely outside validation?
