PURPOSE OF REVIEW: To discuss unresolved issues pertaining to aetiology and diagnosis of isolated left ventricular noncompaction. RECENT FINDINGS: Left ventricular noncompaction may be sporadic or familial and is linked to mutations in mitochondrial, cytoskeletal, Z-line, and sarcomeric proteins.
