What must survive a registry adaptation?

Suppose a patient-reported measure moves from a waiting-room tablet to a phone link completed at home. What concrete example would show which component preserves the intended mechanism, while the collection channel can change locally? I am looking for the decision rule in that example: what evidence would make the channel an acceptable adaptation, and what change would instead alter the intervention itself?

Suppose scores are intended to inform a scheduled team decision about practice. I'd accept the channel change if comparable patient responses still reached that review before the decision. A home link that routinely delivers scores afterward would remove that opportunity for feedback to act, even with high completion; unchanged outcomes would then leave the mechanism inadequately tested.
