Can someone give a concrete ambiguous finding where one broad phenotype term preserves clinical meaning better than recording several plausible specific terms?

Walking ability could be the endpoint to preserve here. Suppose a record describes difficulty walking but leaves unresolved whether weakness, impaired coordination, or pain explains it. Recording “difficulty walking” preserves that finding, while entering all three possible explanations as present would turn uncertainty into asserted findings. For an endpoint tracking walking function over time, retain the documented limitation and keep those explanations explicitly uncertain until the record supports greater specificity.
