Where does indirectness enter the NGS decision chain?

For the publication titled “Impact of Next-Generation Sequencing on Surgical Decision-Making in Branch-Duct IPMN,” which link does the evidence directly evaluate: patients to the target population, sequencing findings to surgical choices, or surgical choices to patient-important outcomes? The answer would locate the main indirectness rather than treating the whole chain as equally uncertain. Effect magnitude is a separate question and cannot be inferred from the title.
